The SOM Executive Committee (Chairs and Center Directors) adopted, without dissention, the attached policy guidelines relating to relationships of all SOM faculty and employees, students, residents and other trainees to industry, particularly (but not limited to) pharmaceutical, biotechnology and medical equipment and device companies.  This is an important step forward in assuring our patients and the public of the integrity of our faculty, staff and trainees in their interactions with indursty, and of our commitment to always keep the best interests of patients as the guiding principle in our decision making.  Please read and adhere to these policies.

 

Required University Training  

    

HIPAA

HIPAA Privacy Training is a requirement for all part-time and full-time employees (including work-study, graduate students, and postdoctoral fellows).  This training is mandatory. On-line Training is available on the website listed above.  After reviewing this material and completing the test, print screen the test scores page and deliver to Kay Guidry, MCLM 701, or fax to 6-2280 as soon as possible, but no later than 1 week after initial hire date. This on-line training may be completed in less than one hour. If you have any questions, please contact Kay Guidry at 4-3275.

 

Diversity

All faculty members, students, postdoctoral fellows and staff members must complete this two part training within 30 days of appointment.  Please go to the link listed above and follow these instructions and complete both parts of the training.  Your certificate of completion of this course must be forwarded to Kay Guidry, MCLM 701, as soon as you receive this document.

 

Effort Reporting

The following people must review and complete the Effort Report orientation required by the University. This is mandatory for the following: 

All faculty members
Monthly employees paid from grants
Students or Postdoctoral Fellows paid from grants
Administrative Staff working with grants

Once you complete this training and are at the acknowledgment page, please print screen and bring this proof of certification to Kay Guidry in MCLM 701 to complete your personnel file.  Call Kay at 4-3275 if you have any questions.

 

Research Code of Conduct

All new employees, students, and postdoctoral fellows must review the UAB Research Code of Conduct.  Please complete this within 5 days of your appointment to the department.  Once you review the material and receive acknowledgement of your completion, please print screen this page and return to Kay Guidry at MCLM 701-0005 to complete your personnel file.  If any questions, contact Kay at 4-3275.  This is MANDATORY.  Thank you for your cooperation.

 

Patent Agreement

Everyone must complete within 5 working days of appointment and return certification to the Department Business Office - MCLM 701.
